I was in the process of purchasing this truck, the mileage is 26000. The dealership filled the tank with gas which was about ½ tank worth of gas. I drove the truck approximately 5 min then returned to the dealership. The truck was running very rough by the time I got back. This is a Honda dealership, so not sure if they have a certified diesel tech. The manager did say they would repair the truck and offered an extended warranty. One, should I allow the repairs? Two, if so what should I see on the work order regarding the actual repairs?

I certainly would want it repaired and it would be fine to let them fix there mistake. I also think the extended warranty is a great way for you to be able to insure the repairs they do are covered.
As for what they need to show on the paper work. In my opinion as long as it runs good when they give it back it doesn’t much matter what they write. But I would think the wording might be something like Replace fuel filter/s and flush fuel system and fuel tank.
